Please fill this out and give to me on Thursday (info meeting thursday at 7- even with more winter storms expected...) email me the form if you can't make it. I need to know who is planning on going. We'd like all of Gospel and Poinsettia, but others can come as non-performers.

I,____________________________________ commit to attending the ACDFA in Arkansas on March 11-14th. I know I must pay $100.00 towards expenses, and will need spending cash for food during the festival. I testify that I am a student of the College of Lake County during the 2008-2009 academic year. Festival fees, room and travel are paid for by the CLC dance department.
I understand that the purpose of participating in ACDFA is to attend classes, perform and to watch performances. I will learn about other college dance programs, and about dance activity around the country. I understand that I am representing the College of Lake County and will conduct myself appropriately, as a mature, intelligent adult. If performing, I will attend extra rehearsals as needed and do my best to perform at my optimum level. If I fail to do so, I understand that I may be replaced at any time with another student. I may also be barred from future CLC dance activities (including ACDFA) for misbehavior.
Registration status is pending until we receive this form, along with a $100- check, payable to College of Lake County, Dance.
